Health Improvement Practitioner(HIP) - Eileen Cramond
Health Improvement Practitioner provides advice and helps patients develop healthier behaviour and lifestyles. They offer practical support to patients and provides intervention on the following:
- Over 40 NHS Health Checks
- High Risk of Cardiovascular Disease
- At Risk of Diabetes
- Newly Diagnosed Diabetes Advice (patients on metformin/diet controlled)
- Smoking Cessation
- Weight Management - eating healthily and losing weight (if necessary)
- Alcohol Advice - encouraging patients to drink sensibly
- Mild/Moderate COPD - health assessments for patients with mild – moderate COPD (Chronic Obstructive Pulmonary Disease), with a view to improving their health and lifestyle.
- Lifestyle Changes - Encouraging patients to participate in increased physical activity, improve diet, healthy eating - high cholesterol patients etc

Mental Health Practitioner(MHP)- Damon Flannery
Damon is our Mental Health Practitioner. He sees patients aged 16 and over as the first point of contact, instead of a GP.
Damon works every Thursday from 1.00pm - 5.00 pm. Please call the surgery to book an appointment with her.
Musculoskeletal Practitioner (MSK) - Edward Day
Edward is our Musculoskeletal (MSK) Practitioner. He is an advanced practitioner Physiotherapist, with extensive expertise in management of MSK (muscle or joint) conditions. Edward sees patients, aged 16 and over, with (suspected or diagnosed) MSK conditions, as the first point of contact, instead of a GP. and can be accessed directly by contacting the practice’s reception team.
MSKs will be based at Little Lever Health Centre.
Social Prescribing Link Worker
Social prescribing and community-based support is part of the NHS Long Term Plan’s commitment to make personalised care business as usual across the health and care system and to bring additional capacity into the multi-disciplinary team. Social Prescribers are an integral part of the primary care network team, delivering holistic service to patients and improving the health and wellbeing of the local population.
Your Social Prescriber can offer support in these areas:
- Loneliness and Isolation
- Social development
- Financial issues
- Housing concerns
- Family concerns
The eligibility criteria for the Social Prescriber is anyone 18+ who is registered at our surgery, who may need help and support in a particular area.Please speak to staff for more information.
PCN Pharmacist - Sidrah Arshad
PCN Pharmacists help practices by reducing workload for the GPs. They work along side the GPs to help review patients medication, update medication changes from discharge letters and help reduce wastage of medications.
A Pharmacist may call you to review your medication or ask for you to book an appointment with them to discuss your medication in more detail.
Health Visitor - -Lyndsey Leatherbarrow
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
Paramedic - Heidi Greally
Heidi is an experienced Paramedic. She assists the duty team with assessment and treatment of acute medical problems and carries out home visits.
